Care Coordination
Recovery and behavioral health treatment often involve more than appointments. Clients may also need help with transportation, insurance questions, referrals, medication follow-up, housing or employment resources, legal system requirements, or communication between providers.
Care coordination at Solstice Counseling & Wellness Centers helps clients navigate those moving parts so treatment is more accessible, organized, and realistic.
Connecting the Pieces of Care
Our team works with clients to identify barriers that may interfere with treatment and recovery. That may include difficulty getting to appointments, uncertainty about benefits, challenges following through with referrals, or the need to coordinate with outside providers and community partners.
When appropriate and authorized, Solstice can communicate with referral sources, courts, probation, medical providers, hospitals, family members, and other supports.
Support That Fits the Treatment Plan
Care coordination is connected to the client’s clinical goals. It may support engagement in counseling, medication management, substance use treatment, recovery planning, or step-down care after a higher level of treatment.
The focus is practical: helping clients stay connected, reduce avoidable gaps in care, and move toward stability.
