Drug Abuse Screening Test
This form provides a preliminary self-assessment tool used by Solstice Counseling & Wellness Center to help individuals evaluate their drug use habits. The screening consists of target questions regarding multi-drug use, the ability to stop, blackouts, legal issues, withdrawal symptoms, and family or medical neglect.
- Scoring Metrics: Results are tabulated on a scale from 0 to 10 to identify the severity of potential substance issues.
- Interpretation: A score of 0 indicates no problem , 1–2 signifies a low-level concern , 3–5 marks moderate problem use , 6–8 points to a substantial problem , and 9–10 flags a severe level of problem use requiring intervention.
