To help you prepare for your outpatient evaluation, please bring two forms of ID, your insurance card, and a list of current medications and healthcare providers for the approximate two-hour appointment. Our admissions team guides you through the entire process, including insurance verification and setting up an individualized, 3-to-6-month treatment plan designed for long-term healing.

Outpatient Admission Tips

The following tips can help you prepare for your outpatient evaluation appointment. We understand the initial phone call can be difficult to make. Our Admissions Team is devoted to ensuring a seamless admissions process. This comprises of a confidential pre-admission assessment to collect all required medical and clinical information. We will personally answer your questions regarding transportation information, financial arrangements, as well as assist with insurance verifications. Our team will guide you every step of the way.

What to Bring

Please bring two forms of identification, your insurance card, your driver’s license, and the names, addresses, and phone numbers of your medical doctor, pharmacy, therapist, psychiatrist, and any other providers we may need to coordinate with regarding your care, such as a probation officer. In addition, bring a list of all medications and dosages.

This evaluation will take approximately 2 hours.You should plan to start our group program the day after your evaluation for three to five days per week based on the level of care indicated by your assessment.

Length of Stay

The actual length of treatment is based on individual progress in treatment. The average length of treatment is between 3 to 6 months.

Call our admissions team at 844-4-SOLSTICE if you need to cancel or reschedule an evaluation appointment. We require 24 hours notice for cancellations or requests for rescheduling. Without this notice, a new evaluation time may not be immediately available.
